2024/2025
Annual Report

CEO

Message from our Board Chairs and CEOs

This past year marked both a bold leap into the future and a proud reflection on our past.

In 2024, Southlake celebrated its 100th anniversary, commemorating a century of care, innovation, and deep connection with our communities. From humble beginnings as York County Hospital, Southlake has grown into a vital health system serving some of Canada’s fastest growing and aging communities.

Our centennial celebrations brought people together in ways that define Southlake’s immense pride for people, place, and performance. Kicking off at the Nature’s Emporium Run for Southlake, the anniversary campaign was an opportunity to honour our past while charting our future by bringing communities together through a vendor market, Movies Under the Stars event, heartfelt staff celebrations and other moments of reflection recapping milestones throughout our history.

And what comes next is transformational.

Our Impact

Southlake Health and Southlake Health Foundation have made a meaningful difference in the lives of thousands of patients and families across our region. From investing in lifesaving technology to expanding access to specialized care, every milestone reflects our commitment to delivering leading edge care, close to home. None of this would be possible without the generosity of our community and the dedication of our medical staff, staff, and volunteers.

Reflecting on our Pride for People, Place, and Performance

This year’s stories reflect Southlake’s pride of people, place and performance. These pillars shape how leading edge care is delivered and how Southlake Health continues to grow to meet the needs of patients and families.

The stories highlight the skill and compassion of teams across Southlake, our strong partnerships, and the support of generous donors and local communities. Each story is a reminder that progress is possible when we work together.

For 100 years and counting, Southlake Health has built healthy communities through leading edge care, innovative partnerships, and amazing people. To chart the next 10 years of care, Southlake’s new strategy will leverage its expertise to build a Distributed Health Network that provides leading edge care, closer to home.

Anchored by its Davis Drive site and existing community-based locations, Southlake will work with partners to establish additional footholds across the municipalities it serves to tackle hallway healthcare by developing a Distributed Health Network. Key to this will be the creation of a second hospital site and the redevelopment of the Davis Drive campus. Southlake will also expand beyond its walls all together, with a focus on virtual care and by leveraging learnings from its successful Southlake@home program.

By evolving into a Distributed Health Network, Southlake will improve outcomes and experiences for patients, families and caregivers, while also enhancing satisfaction for its teams, addressing health equity concerns and ensuring sustainability. Fueled by transformative philanthropy from our generous donors, in the decade ahead, Southlake Health will continue to focus on what we do best - providing leading edge care to the communities who rely on us.

With special thanks to our Boards of Directors

Southlake Health and Southlake Health Foundation are guided by volunteer Boards of Directors made up of dedicated leaders from across our vast catchment area. Their support helps shape the organizations’ direction and commitment to delivering high-quality care.
